Family Support Home Habilitation
The family Support Home Habilitation Program provided is designed to include linkage/referral, follow-up, monitoring, family member training and adaptive equipment assistance to people living at home, with families, who have a developmental disability. Training and reinforcement is offered to parents, siblings and caregivers designed to augment or improve their knowledge of and the ability to care for their family member at home. Home Habilitation also offers training in activities of daily living which enable individuals to increase their level of independence and improve the quality of their lives.
Family Reimbursement

This service is provided as an enhancement to our existing programs to provide financial assistance to individuals and families in the form of reimbursement for items/services that are not typically covered by Medicaid, Medicare or Insurance. Applications are accepted and reviewed on an as needed basis. Examples of requests include assistance with registration and accommodations to National Self-Advocacy Conference, crutch tips, orthopedic shoes, Special Needs camp, fire extinguishers, etc.

Medicaid Service Coordination
Medicaid Service Coordination (MSC) is a Medicaid State Plan service provided by OMRDD which assists persons with developmental disabilities and mental retardation in gaining access to necessary services and supports appropriate to the needs of the individual. MSC is provided by qualified service coordinators and uses person centered planning process in developing, implementing, and maintaining and Individualized Service Plan (ISP) with and for a person with developmental disabilities or mental retardation. MSC promotes the concepts of choice, individualized services and supports, and consumer satisfaction. MSC is designed to help consumers strive for the highest quality of life. The focus of MSC is an assisting each person to achieve his of her unique goals and desires in terms of having a home, a job, a circle of friends, and enjoyable leisure activities and access to medical and clinical services. Maximum caseload for residing with family or alone is 25 per service coordinator. In addition to Core training, service coordinator are requires to attend 15 hours of professional developmental training each year.
Home and Community Based Waiver Services
Environmental Modifications / Adaptive Technologies    
Examples include: Access ramps, bathroom modifications, wheelchair lifts, etc.

Residential Habilitation
Residential Habilitation is usually provided where a person lives. The services may be provided at any time of day. Some examples of goals the individual may wish to work on are: cooking, dressing themselves, toilet training, cleaning house, budgeting, appropriate social behavior, personal hygiene, and planning menus.

Day Habilitation without Walls
Day Habilitation allows for services to occur that are desired by the consumer. We are attempting to meet the goals and desired outcomes of the consumers rather than making the consumers fit into existing programs or services. Day Hab without Walls takes place 51% of the time outside the individual's home. Some examples of goals the individuals may wish to work on are: using public transportation, safety skills within the community, shopping skills, utilizing community resources such as the library, appropriate public behavior, how to order from a menu, etc.

Family Education and Training
This service may be provided to the families of consumers enrolled in the Home and Community Based waiver who are under 18 years of age. The purpose of family education and training is to enhance the decision making capacity of the family unit, provide orientation regarding the nature and impact of developmental disabilities upon the consumer and his or her family, and inform them about service alternatives. Family education and training is distinct from service coordination in that the purpose is to support the family unit in understanding and coping with the developmental disability. The information and knowledge imparted in family education and training increases the chances of creating a supportive environment at home and decrease the chances of a premature residential placement outside the home. Education and training topics may include: coping with grief and loss, sibling issues, sleep issues, socialization and recreation options, housing options, the extended family as a resource, future planning, navigating the special education system, preschool special education, etc.

Plan of Care Support Services
Beginning March 2000, Medicaid Service Coordination was established. MSC is an optional service. In order to continue to meet the service plan and level of care eligibility requirements in New York State's waiver agreement with HCFA, consumers enrolled in the HCBS Waiver who choose not to receive MSC will receive Plan of Care Support Services. Plan of Care Support Services will be provided only to consumers whose decision to receive MSC has been approved by the DDSO.

Clinics

CP Specialty clinics are held on a monthly basis. The clinics specialize in orthopedic care for the developmentally disabled. These clinics are unique in the fact that we are able to offer individuals and families a more comprehensive approach to the services available. We provide the opportunity for Physical Therapists, Occupational Therapists, Orthotic Consultants, Medical Supply Consultants, Service Coordinators and Nursing staff, at the time of clinic.

NNYCPA is an Article 28 Diagnostic and Treatment Center operating under the auspices of the New York State Department of Health.

Due to the design of the clinics, acute problems are not addressed here.

Home Service
This program provides a regular and continuous relationship between the agency and the consumer and his/her family. It is further intended to provide an additional resource to other agencies, organizations or individuals that may be working with a disabled child or adult. The Home Service staff person is able to provide assistance in securing adaptive equipment or assistive devices for the home. The other areas that are addressed include advocacy, referral and follow-up services. The agency staff person is knowledgeable of general and specialized services for individuals with developmental disabilities. This would include medical, educational and vocational resources available and the procedure for obtaining these services. Continual contact is maintained with individuals and their families whether or not essential referrals are needed.
Case Management
The Family Support Case Management program provided regular contacts with the individual, primary caregiver and service providers to assist with the planning for and accessing of desired services and supports. Services include assessment of service needs, service planning, using a person-centered approach, and coordination, linkage and referral, follow-up monitoring and advocacy.
Home Care/Voucher Respite
In cooperation with the Central New York Developmental Disabilities Service Office, NNYCPA is able to offer families the opportunity to hire an individual of their choice to care for their developmentally disabled family member in the home. NNYCPA reimburses the family the cost of the services rendered.
Guardianship Assistance:
Financial assistance is available to families of Lewis County who need to establish guardianship for their family member. This assistance is in the form of family reimbursement with a cap of $350.
Horseback Riding:
Financial assistance is available to families of Lewis County who have a child over the age of 5 who is interested in benefiting from horseback riding. This assistance allows for 10 individuals to receive 10 lessons.
